Is coffee low FODMAP?
Black espresso and brewed coffee are low-FODMAP. Trouble is what you add: milk, syrups, oat milk, and chicory-blend instant coffees. Caffeine itself can also irritate IBS independent of FODMAPs.
1 espresso, 1 cup brewed black coffee. Lattes only with lactose-free or almond milk.
Pure coffee — black espresso, drip, French press, cold brew — is low FODMAP. The problem is almost never the coffee bean itself.
The two real problems
1. The milk
A standard 12oz latte has ~250ml of milk = double the lactose threshold. If lactose is your trigger, this is the culprit, not the coffee. Switch to oat milk (small portion only — see below), almond milk, or lactose-free milk.
2. Caffeine itself
Independent of FODMAPs, caffeine stimulates colonic motility in many people. If you tend toward IBS-D (diarrhea), strong coffee on an empty stomach can trigger urgency within 30–60 minutes. This isn’t a FODMAP problem; it’s a separate irritation. Solutions:
- Drink coffee after food, not on empty stomach
- Switch to half-caf or decaf — most decaf is also low-FODMAP
- Try cold brew — lower acid, often gentler
Plant milk gotchas
- ✅ Almond milk — low-FODMAP at 1 cup
- ✅ Lactose-free milk — unlimited
- ⚠️ Oat milk — only 1/4 cup is low-FODMAP. A standard latte’s worth (1 cup) is over the fructan threshold. This is the most common surprise for IBS sufferers who switched to oat milk thinking it was “gentler.”
- ❌ Soy milk made from whole soybeans — high in GOS
Other watch-outs
- Chicory-blend instant coffee (some Asian and European brands add chicory) — high in fructans
- Flavored syrups — sugar-based ones are usually fine; agave-sweetened are high
- “Mushroom coffee” — depends on brand; reishi/lion’s mane are not high FODMAP, but check
- Bulletproof / butter coffee — fine for FODMAPs, but the fat can trigger gallbladder symptoms in some
What about coffee on phase 1?
Some FODMAP guides say “limit coffee in phase 1” — that’s about caffeine sensitivity, not FODMAP content. If coffee triggers you regardless of milk, try cutting back to 1 cup/day during elimination and reassess in phase 2 along with reintroduction.
